Payment and Cancellation Policy
At Fortis Physical Therapy and Pelvic Health, we strive to offer flexibility while ensuring our services are accessible and effective. Our cancellation policy requires 24 hours notice to avoid a fee. For payment, we accept a variety of options and ask that all charges be settled at the time of service unless otherwise arranged. At Fortis Physical Therapy and Pelvic Health ("we," "us," or "our"), we are committed to providing exceptional care and maintaining a smooth administrative process. This Payment, Cancellation, and No-Show Policy outlines our expectations regarding payments and appointments to ensure clarity and mutual respect.

Accepted Payment Methods

We accept cash, credit/debit cards, and HSA/FSA cards.

Payment is due at the time of service unless other arrangements have been made in advance.

Insurance

If applicable, we will file claims with your insurance provider as a courtesy. Any co-pays, deductibles, or non-covered services are your responsibility and must be paid at the time of service.

It is your responsibility to verify coverage and benefits with your insurance provider before your appointment.

Outstanding Balances

Any outstanding balance must be paid in full before future appointments can be scheduled unless a payment plan is established.

Cancellation and No-Show Policy

Appointment Cancellations

We require at least 24 hours’ notice for cancellations. This allows us to offer the appointment slot to another patient.

Cancellations made with less than 24 hours’ notice may be subject to a cancellation fee of $50.

No-Show Policy

A "no-show" occurs when a patient misses a scheduled appointment without prior notification.

No-shows will incur a $75 fee, which must be paid before scheduling future appointments.

Late Arrivals

If you arrive late for your appointment, we will do our best to accommodate you within the remaining scheduled time. However, the full session fee will apply.

Arriving more than 15 minutes late may result in the appointment being marked as a no-show.

Exceptions
We understand that emergencies and unforeseen circumstances arise. If you are unable to provide sufficient notice due to an emergency, please contact us as soon as possible. We may waive cancellation or no-show fees at our discretion.
